The entrance to Azoturan Fortress is an Abyss Gate inside the Lepharist Research Center.
Acestes will auto-fly you to Peitho in Azoturan Fortress who, after a short cutscene, will auto-fly you into the Azoturan Fortress Interior. From there you need to fight your way down to the Azoturan Fortress Sublevel where you will find Betrayer Icaronix in the room where the Deity General would normally stand. The room will have many other mobs in it, and you must clear them all first, or else Icaronix will call them all to him when attacked. Shortly into the fight Icaronix will transform into an RM. He will have enormous health and hits very hard. This is a long, steady fight.

In the first cutscene Icaronix implies that you also serve Lord Lephar, or that you did, and in the final cutscene after his defeat, Icaronix predicts that you will kneel at Lord Lephar's feet.
