I have a question about beta test phase 3, I used to have a FFXIV active account but I quitted for long time.
Do I have to sign up for beta test? or I will be automatically get into to phase 3 as well?

As far as I know, you'll be invited no matter what. You should still sign up regardless, assuming you actually want to test the game before that point.

Phase 3 and 4 are "Open" beta. You won't need to be invited to them - anyone will be allowed to participate. This is also usually when they stress test the severs =)

Phase 3 is not open beta. It is, however, the beta phase where all former account holders will be invited.
From page 11 of the FFXIV 2.0 Beta Roadmap pdf...
"All FFXIV service account holders qualify as testers in phase 3."
So if you ever activated an account for FFXIV, you have a service account and will be in for phase 3 at the latest. As was stated above, you should still apply if you want a chance to get into an earlier phase.

Yeah, I do have a service account but I applied for the beta anyway.
