I shouldnt be the only one keeping my account safe. SE should make it harder for them to login from lets say... the other side of the world... when clearly i do not play from that location... atleast not let them login from there without an e-mail confirmation or something.
I dont use the same password twice on any website, i have an second email account just for FFXI and FFXIV. Plus i dont use my gaming computer to look for ****. Not to mention i dont login on my FFXI(V) email from anywhere other than my gaming rig either. If that isnt enough to not get hacked, SE is doing something terribly wrong.
I've reported every single RMT so far i've seen shouting or gotten a tell from, three days later, most of them are still around. It's gotten to the point where i'm slowly giving up. Which is probably exactly what they want, but still. SE really needs to step up and stop the blatant RMT abuse in this game before it ruins it. Not economy wise, but fun wise. Cant even get into town now without having to blacklist yet another 2-3 people first.

The security token isn't absolutely necessary if you follow the practices that you do, but not everyone cares to go to those extremes. Two factor authentication in the form of a security token gives the masses 99%+ assurance that their account is secure.
Keep in mind, it can increase
your security also. Call it a safety net. No matter how careful you are, sometimes sh%t happens.
I do agree with you on the IP check, but I was under the impression that they had implemented this to some extent, locking accounts temporarily.
